How do I look up a nursing home's star rating?

Search the facility by name, city, or ZIP code above. Every Medicare- or Medicaid-certified nursing home has a CMS Five-Star rating (1 to 5) built from health inspections, staffing, and quality measures. The report card shows all four ratings next to the state and national averages and as a percentile against the other rated facilities in the state.

What is a good nursing home rating?

A 5-star overall rating means "much above average" and a 1-star means "much below average." Because CMS assigns inspection stars relative to other facilities in the same state, the percentile on the report card is the fairest comparison: top 10% in the state is excellent, while below 25% is a warning sign regardless of the star count.

How can I see a nursing home's inspection violations?

The report card lists every health citation from the last three inspection cycles, including complaint investigations, with the date, what inspectors found, the scope and severity letter decoded, and whether it was corrected. Citations rated G through L involved actual harm or immediate jeopardy and deserve a direct question to the administrator.

What does the abuse icon on a nursing home mean?

CMS adds an abuse icon when a facility was cited for abuse that led to harm in the past year, or for abuse that could have led to harm in each of the last two years. The report card explains this flag along with Special Focus Facility status, overdue inspections, and recent ownership changes.

How often is the nursing home data updated?

CMS refreshes Care Compare monthly and we reload the full dataset quarterly. Every report card shows the CMS processing date it was built from. Inspection results can lag reality by a few months, so always ask the facility about anything recent.
